Please attach all original receipts and cheque column. Union officers cannot sign own credit card expense vouchers. Please submit to Signing Officer for signature.

Acceptance of Nomination Form

This form is to be completed by members unable to attend a meeting where elections will be held for elected positions, conferences, conventions, educational, etc.

To be nominated, members must be in good standing. Members signing as witnesses must also be in good standing. If you require more information about your standing, please contact the secretary at secretary@cupe905.com

1. Complete the form, including signatures.
2. Submit the form a. Email a copy with signatures to secretary@cupe905.com at least 1 hour PRIOR to the start of the scheduled meeting OR b. Have the form presented at the meeting by a member in attendance.
